Just call (209) 558-8900 and we'll send you a simple one-page application. You can also pick up an application at any Stanislaus County Health Services Agency locations.

For additional assistance, you can schedule an appointment with a patient family services specialist in our Modesto or Turlock locations by calling (209) 558-7393. Staff may also assist you with applications for other health programs when appropriate.
